No, it really doesn't. It would force the site to follow app store rules, which would only be possible by implementing all kinds of massive censorship.
I mean, aside from being able to distribute an android app entirely independently of the play store... This isn't even remotely true!
Tumblr exists, Twitter exists, Web Browsers exist, Discord exists. You can find all sorts of 'questionable' content easily with the click of a button.
Despite this, all those are still on the app store without any censorship. In order to evade the platform rules, you just need to have 18+ hidden by default, and only enabled by a setting. That's how tumblr, twitter, and other websites have done it. There's also several third party AO3 apps on the play store that were not removed nor 'censored' by anything, so your point is invalid.
The more likely reason is that the OTW has limited resources to manage AO3 as a whole, and developing an app is just going to be... to expensive, too much resources overall, and too much hassle to maintain it.
