Saint Luke Hospital, Tokyo
Patient’s name: Midoriya Izuku
Age: 28 years old
Occupation: Costume Designer/Engineer for Hatsume Inc
Status: Mated/Married
Sex: Male
Secondary Sex: Omega
Green haired male omega with freckles and curly hair is nervously waiting for the diagnose of his health in the doctor office.
The doctor is still reading the result of his X-Ray and blood work with unreadable expression.
“Midoriya-san” he says.
“Yes?” Izuku replies with so much anticipation in his voice.
He is looking forward for the results. He actually came to the hospital to discuss his plan of adding a pup to his household. Being in the pack with his high school sweethearts, Izuku promised each one of them to give them a pup and so far, he has given 4 pups. Iida, the Prime Alpha of his pack will have his turn in his next heat which will be in a month time. Izuku really hopes that there won’t be anything wrong with his body because he really wants to get pregnant again.
“I don’t want to be the bearer of bad news but do you by any chance, feel more tired these days? The fatigue is unbearable or pain in your abdomen area?”
Izuku swallows his saliva, he didn’t expect that kind of comment from his doctor. Could it be that there something wrong with his body?
“Just a little, work has been keeping me busy since our business has soared ever since we won the awards for best support company in hero ranking last year. The pups also have been really active and cheeky these days” Izuku lets out a nervous chuckles, trying to hid his anxiety.
“I see, though the results from blood works and X-Ray are mostly positive, I am curious with this lump in your lungs. Say, do you cough often these days?” as he shows him the results of his blood work where is leucocytes level is staggering high and the spot in his lungs via the X-Ray.
“I highly suggest for you to take more thorough check-up with our MRI” he continues.
“Are you sure, sensei? Do you think it might be something dangerous?” Izuku asks worriedly.
“Well, I hope that it might be just a polyp and the leucocyte level is due your stress level but we can never be sure” he answers.
Izuku looks down as he checks his phone background.
Ten-chan, Shouchan, Kacchan, Hito-chan, Ei-chan smiling with their pups, Katsuma, Eri, Kouta and Mahoro.
Izuku can’t leave them alone in this world, he wants to see his pups graduating college, successful in whatever fields they choose and maybe getting married later.
He can’t die. He refuses to die.
“God, please just make it a false alarm” as he silently prays.
His husbands, he can’t leave his husbands. Bonds between Alphas and Omega in a pack is sacred. He can’t bear the thought leaving his alphas by themselves.
Especially, Ten-chan. God, he needs to fulfil his promise to Ten-chan.
“I consent for the test” Izuku surrenders.
“Good, can you please sign this form? A nurse will guide you to the MRI room soon” the doctor puts a piece of paper in front of him. Basically, a consent letter for MRI.
Izuku tries to hold back his tears, he keeps on praying silently in his hears.
Please, make it a false alarm, as he keeps telling himself.
A friendly Beta nurse then guides him in MRI room where he changes into a hospital gown and instructed to lay down on a table where it will automatically move him inside the machine for further scan.
The MRI is pretty fast after that and the doctor sets him for another appointment the next day after working hour at Hatsume Inc. Izuku nods with an empty expression.
His steps feel so heavy after that.
He still feels surreal and thousands of scenarios are playing in his mind.
Not until when he has the time to sit at the train then he begins searching for possible ailments online. Most of the search results are horrifying but it gives him hope as most of it are curable. But it still doesn’t make him relaxed.
Izuku decides not to tell his family until he gets the results.
He walks straight to his home in the posh neighbourhood in Tokyo.
The security system scans his eyes and the gates are automatically opened. He walks to the door with a heavy heart as he scans his fingerprint and the doors are unlocked.
He takes a deep breath while putting up a brave front.
“I’m home” as he enters his home with a wide smile.

Izuku POV
I need to put up a brave front.
Yeah, that’s it. Since the results most likely either it is curable or just a common infection? Maybe a flu, yeah, that must be it.
“In the 5 stages of grieve, I think you’re in denial stage now” his inner thoughts say.
“Shut up” Izuku thought. No way I’m gonna die. My husbands and pups need me more than ever, I can’t die. I’m not allowed to die and I don’t want to die! As he battles with his own inner thought.
“Mama?” a 6-year-old boy with curly blond hair and freckles is looking at him in worried.
“Katsuma” Izuku instantly putting up his happy wife face and smiles at him.
“Aren’t you going to give me a hug” Izuku teases.
Katsuma seems confused at how Izuku suddenly changed from looking sad to happy but knowing his mother can be enigmatic sometimes, he just shrugs it off by hugging him.
“I miss you” Izuku says. Know saying this kind has a different weight he realises. He doesn’t know if he needs to count down until.. STOP!
“Mom?” Katsuma’s big eyes are looking at him worriedly.
Izuku realises that he must have given some weird impression. He just tightens the hug while rubbing his backs.
“Nothing” he mutters.
“Huh?” Katsuma sounds confused.
“Katsuma, I love you so much. You know that, right?” Izuku tries to contain his tears.
“Mama? Are you crying?”
“Huh? Katsuma made mama cried!” suddenly another boy with spiky black hair appears from the door that leads to the living room.
“Nooo! I didn’t” Katsuma argues.
“Katsuma! You did it again!” this time a girl who look like Katsuma sprints up from the stairs just to pint finger at him.
“N-no…I didn’t make him cry..” this time Katsuma starts to sniffle.
“Hey! Don’t bully Katsuma-kun” a young girl with a horn and silver hair appear from living room with a tried looking guy.
“Hoo, another showdown” he lazily grins.
“Hito-chan” Izuku rolls his eyes.
“Come on, pups. If you wanna hug mommy” Izuku smiles at his pups. Eri, Mahoro and Kouta who instantly tackles him into a hug.
“Oh” Shouto later comes up from the kitchen as he snaps the moment using his smartphone.
“Ohhh, a group hug. Lemme join!” Eijiro who just came back from his patrol suddenly enters the door and witnesses the adorable scene.
He then basically hugs them and lift them up from the floor.
“Ei-chan! Hahahaha” Izuku laughs.
“Dada! You stinks!” Katsuma covers his nose.
“Ouch, that hurts!” Eijiro pretends as if he got shot.
“Can you extras shut up and eat your food! Ei, you stink! Go get showered!” Katsuki screams from the kitchen.
“Yes, daddy!” the pups hurriedly march into the dining room, leaving the pouting Ei and the rest of their dads and mom.
“The pups are so energetic” Izuku coos.
He then glances at Ei. “Welcome home, Ei-chan” as he kisses him on his lips.
“Izu, you’re not annoyed with my scent?” he asks. Ei probably has the most potent smells among his alphas and many fellow alphas don’t like it. Most omegas even find it unpleasant.
Izuku on the other hand finds it comforting. Like the smell of asphalts after rain with musky undertone.
“Nope, I actually find it really attractive” he winks cheekily which made Ei blushes.
Shouto pouts “Izu, I haven’t got my kisses yet” he whines.
Izuku sighs as he walks towards him and gives him a kiss.
“I’m home” he says.
“Welcome home, angel” Shouto smiles.
“Hey, hey. How about me?” this time Hitoshi protests.
My oh my, his alphas are worse when it comes to sharing but Izuku won’t have it any other way.
“I’m home” suddenly the doors opened and Tenya is there bringing some souvenirs.
“Ten-chan!” Izuku practically throws himself into his embrace.
“Hahaha, Izu-kun” as Tenya kisses him on his lips.
“Tch, I haven’t got mine” Hitoshi whines.
Izuku laughs seeing his how pouty Hitoshi can be before giving him another kisses.
They then make a beeline to the dining room where all the pups have been waiting for dinner.
It is a normal quality time with them. It somehow helps him forgetting his predicament today but it stays at the back of his mind.
He can’t help but feel guilty for not telling them and that he may not be able to give Tenya pups.

That night, Izuku tries his best to mask his anxiety as Kacchan has kindly cooked each of their packmates’ favourite food. Today, Katsuki also had an off day which he spent cleaning the house, dealing with the pups and cooking.
For Izuku he made a Katsudon, for Tenya Beef Stew, for Shoucchan Zaru Soba (from scratch), for Hitoshi he made a cheese in hamburger (like Eren) and for himself, he made a super spicy curry rice.
As for the pups, he made chinjao rosu or beef and green capsicum stir fry for Katsuma, chicken pot pie for Kouta, beef and broccoli stir fry for Mahoro and yakisoba or fried noodles for Eri. Today Katsuma cried a lot and Mahoro was a brat so Katsuki ended up punishing them by making a food with veggies that they don’t like.
“Mahoro, eat your veggies!” Kacchan barks at his daughter who replies by sticking her tongue.
“Mahoro-chan, you won’t become a hero if you don’t eat your veggie” Eri, as a good sister reprimands her petulant younger sister.
“But Eri-nee, broccoli doesn’t taste good” she pouts.
“But it is rich in nutrients and it will help you to grow strong like daddy” Izuku smiles.
“Daddy likes broccoli?” she asks curiously.
“Tch, of course!” Kacchan snickers.
“Yeah, I can see that” Hitoshi smirks as he sends a glance to both Kacchan and Izuku, implying that Kacchan has been liking Izuku ever since they were children.
“Hicchan!” Izuku flusters.
“Shut it, brainwash! There is no way I like that stupid nerd when he was a child!” he seethes.
“Sure, whatever helps you sleep at night” Hitoshi rolls his eyes.
“Damn you” Kacchan grits his teeth.
“Hey, hey Katsuki. Don’t say bad words in front of the pups” Eijiro pats his shoulder while grinning.
“Agree, it is most unbecoming of a good parent to say profanities in front of growing pups” Tenya karate chops the air.
“Grow up, Katsuki” Shouto just mocks him with a monotone voice while continuing slurping his soba without a care in the world.
Izuku lets out a chuckle seeing how the family interact.
“What’s funny, Deku?!” Kacchan barks.
“No, it’s just… I love you all, that’s all” as he spontaneously says it.
“Hahahaha, I guess this is what we call happy family” Eijiro lets out a laugh which makes everyone in the table laughs in unison, except Kacchan who pouts. Even Shoucchan lets out a chuckle.
“Mahoro-nee, I can eat your broccoli if you want” the sweet angel, Katsuma offers.
“Katsuma, you better eat your capsicum before offering to eat other’s veggies” Kouta teases.
“B-but…” Katsuma stutters.
“Kouta is right, Katsuma. Eri-nee has said that you won’t grow up big and strong like daddies if you don’t eat your veggies” Izuku chimes in.
Katsuma looks like he wants to cry as he picks up the green capsicum slice from his plate of chinjao rosu (beef and green capsicum stir fry) and eats it.
“Good boy” Izuku pats his head and Katsuma beams at him.
Mahoro pouts and hurriedly eats her own veggies “Mommy, look! I can also eat my veggies” she said proudly before choking on her broccoli.
“Stupid brat!” Katsuki instantly helps by patting her back to help her release the broccoli.
“Barf!” finally she managed to puke the broccoli.
“Oh my god” Izuku hurriedly hugs his pup.
“Gosh, don’t scare me like that” he scolds.
“I’m sorry, mommy…” as she cries.
“It’s ok, my love. It’s ok… Please be more careful next time. Always chew 30 times, ok?” he continues by rubbing circles on her back.
After the scare, the rest of Izuku’s husbands tuck their own kid into the bed before going on their own room.
In this house, each alpha has their own room and usually Izuku has the choice to spend the night with the alpha or sleep in his own room.
They also are keeping track of their own rut’s schedule when Izuku will help his husband and they will help him during his heat which coincidentally last for 5 days each 2 months.
As for today, Izuku will spend the night with Tenya.
Izuku himself is currently pandering his own thought while laying down Tenya’s king sized bed. He is concentrating not to release any distress pheromone or his mates will worry.
“Oh, you’re here” Tenya just finishes showering as he exiting the ensuite bathroom only wearing a bathrobe.
“Yes… I’m so tired” Izuku lets out a sigh.
Tenya smiles at him as he lays down next to him.
“Is it ok if we just cuddle and sleep?” Izuku feels guilty but he really is not in the mood after hearing the news.
“Of course,” Tenya ruffles his hair.
Izuku snuggles closer as he rests his head on Tenya’s well-built chest.
“How was your day?” izuku asks.
“Same old, chasing robbers, mentoring new sidekicks. Oh, Tensei said hi, by the way” he says.
“Been awhile since we had a lunch with him, maybe we can set up a lunch your family soon. I mean, my heat is two weeks away” Izuku teases as Tenya blushes.
“O-of course! I’ll let them know” he tries to regain his composure.
Izuku chuckles looking at how adorable his big alpha husband is.
“I love you, Tenya” he breathes in Tenya’s comforting pine scent as he closes his eyes.
Tenya kisses Izuku’s forehead before sang “I love you too, my omega”.

The next day, Musutafu Municipal Hospital
Izuku is looking at the results while tears are flowing from his eyes.
“I’m really sorry, Mr. Midoriya” the beta female doctor, Akimoto-sensei, apologises.
“C-could you explain it once more about this ailment?” Izuku asks.
“Certainly. It is called malignant cells tumor in quirk factor. It seems like you actually have a quirk but it is dormant and the quirk factor cells are multiplying rapidly and begins to attack healthy cells in your lungs. It may be mistaken as a lung cancer which is easily treatable in this day and age but it is not” she explains.
“How much is the chance of survival?” Izuku bravely asks.
“It is a rare type of cancer, the data is pretty scarce but roughly 5%” she looks at him sympathetically.
“I see…. What are my options?” he asks.
“Since this disease is fairly new and rare, there are no 100% effective treatment yet. However, a chemotherapy will help prolonging your life span” she says.
“Ok, which stadium?” izuku is sniffling now. The doctor gives him a tissue and puts the tissue box next to him.
“Thank you” he says.
“No problem. As for your question, Stadium 4” she answers. Izuku knows what that mean, it means that the cancer has spread.
“How much longer do I have?” he finally asks.
The doctor lets out a sigh. It is never easy to tell bad news.
“A year maximum” she says.
