When it appears that Sam is put into a coma during a hunt Dean makes a heartfelt confession that he's been holding onto for years, but the problem is Sam isn't unconscious and hears every word his brother says. So now what?

**Blinding white light was the last thing Sam saw as his body crumpled to the ground beneath him. His mind was still functioning perfectly but for some reason his body was not responding to the commands that he was sending for it to get up off the ground. He could feel the pain in his shoulder as it hit the ground and the dirt beneath his face as he lay there but noting was moving, no matter how much he willed it to.
“Saaamm!” He heard in his brother’s terrified yell which actually comforted the younger man. Dean was coming, he’d fix whatever this was. His brother could be many things, a lot of them irritating, but he was the one person in the whole world who never let Sam down.

It didn’t really make a difference when it began anymore because the damage was done. John took another drink and closed his eyes to the blood on the floor and the sight of his bruised knuckles staring back at him as he lifted the bottle. Maybe he deserved this for the way he’d made them live all these years, for what he’d done tonight. Maybe this was all his fault, but could anyone blame him for his reaction? Seeing his two sons scrambling for their clothes as they tried to say it wasn’t what it looked like as the bottle of lube sat right there on the bed, mocking them all with the sent of sex still hanging in the air had nearly broken John’s mind. And maybe if Sam hadn’t opened his mouth…. If he hadn’t taunted John… Maybe.

Figuring out your brother has incestuous feelings for you is a head trip, but finding out your brother sold his soul to bring you back from the dead is crushing. Gut wrenching, overwhelming, devastating, and enough to break even the strongest person. Finding out Dean only had a year to live before spending an eternity in hell damn near killed him again but it also made Sam start to wonder.
Watching as Dean went through his little bucket list while trying to live his best life with food and sex and killing monsters got Sam thinking as he knew there was one thing Dean wanted more than anything and it wasn’t on that list. Could Sam give that to his brother?

When Dean has his face to face with Death to deal with the Mark he is actually presented with two options, one of which is to bind his soul with his soulmate to distribute the load between the two and therefore be able to manage the Mark's effects. Dean thinks it's too much burden to place on his brother but Sam has a different reaction. Guess who wins that argument.
“Wait a minute… I can’t do that to Sam, I can’t ask him to help me carry this burden and have it destroy him as well.”
“Then the only other option is that I would need to relocate you somewhere far away… not even on this earth… where you’d still be alive but no longer a danger to yourself or to others?”
“Okay, then that… I choose that.” Dean said and tried not to think of what that choice would mean, that he’d never see his brother again.
