Dean hits Sam. Dean does awful, demeaning things to Sam, and Sam knows he deserves them. Sam has no choice but to deal with it, because he knows Dean loves him, even if in his own twisted way. Even if he shows it funnily. Dean is the only one who so much as pays attention to or cares for Sam Winchester. So it's okay that Dean needs to hurt him. He's family, and the only support he has left. Plus, it's not like Sam doesn't love Dean back, because God he does. He loves him so much that he takes everything Dean gives him. Everything, because he's selfish.
