John left the war with a mess of a shoulder and no leg, but that doesn't really affect him much later on. it wasn't something that Sherlock brought to attention, but they both knew. The yard did not. (Pretty much John throws his prosthetic at a murderer and that's how Greg finds out he didn't have a leg.)
Idk how accurate this is because I don't have a prosthetic, but I wrote it more for comedy than anything. Loosely inspired by that one guy who was with his wife when someone tried to mug them so he hit him with his cane or prosthetic. Don't know if anyone knows what I'm talking about.

John Watson was a military doctor, a captain, he saw the death of the people around him and met death several times.
OR : John and The Death met many times during the young man's life, whether when he came close to death or when he saw a corpse, the two created a sort of "bond". So when Sherlock jumps from that building, John knows something is wrong.

“Careful with it.” Sherlock’s voice startles him. The violin slips from his shoulders, as John lets it - carefully - drop into the case, together with the bow, before he turns to his flatmate. The man leans against the door frame, wrapped in just a blanket, a smirk on his face.
John smiles awkwardly at him, “Sorry, sorry! I was just putting it away. I… spilled some water, while making tea. On the kitchen table. Wouldn’t want the violin to-“
Sherlock shakes his head and moves towards him, pointing at the violin. “Pick it up.”
Carefully John bends to grasp the violin and he shoulders it once more, letting it sit awkwardly on his bad shoulder.
"You’re holding it wrong,” he states flatly.
John prepares to lower it, an apology ready on his lips. Until this very moment - or maybe it was the night when Sherlock played - John had had no interest in playing the violin. But now that he stands here, the wood against his skin, the instrument ready, he can’t help but smile a little at the detective’s odd look.
