Cas is standing there, but he's covered in goo. He's in no pain. He's not suffering. He's still smiling like they were at the beach they never got to go to. And all Dean can think about is NO.
Suddenly the dungeon has a window, and horrible red wallpaper. He looks around in confusion, and it slowly dawns on him that he ain't in Kansas anymore, Toto.
He doesn't have time to react to that revelation, because there's the sound of a gun's safety clicking off, and then he's face to face with 29 year old him.
OR: Dean travels back in time to when he first met Cas whilst trying to save him from the Empty, and maybe has to come face to face with the fact that Cas's confession isn't exactly one sided

Logan Berry had a superpower, but it was a strange one. He could not make fire in his palms or shoot light beams out of his eyes. He could not create matter in an instant or destroy things with a snap of his fingers. He could not read minds or see into the future. In fact, he lacked every known or discoverable genetic marker for superpowers.
That did not mean he didn’t have a superpower of a different sort.
Logan had the very common, but rarely utilized ability to think... and then to make those thoughts real.

“You are clearly present for a reason, Remus,” Logan chimed in, after another beat had come and gone. “You may be glad to hear that we are all curious as to what that reason is, and why you have not moved from the couch since this morning.”
“Guys, be nice,” Patton chided softly.
All they were met with was another not-statement, this one a bit more petulant than the first. The duke tensed up and rolled over with a tight breath, at last, flopping onto his back and letting his left arm fall over the edge of the couch.
“I’on’feelgood,” he mumbled, his voice uncharacteristically quiet.
