He means it as a joke. It's so absurd that it couldn't be anything other than a joke. And he knew exactly how Izuku would respond. He would roll his eyes and groan and shove him and blush and say, Kacchan, you can't just say things like that!
But that's not what happens. Instead, Izuku's whole face goes paper white before turning bright red again so suddenly that Katsuki's almost afraid he's going to pass out on him. Izuku is silent.
Holy shit.

Keeping their relationship a secret was a mutual decision. There seemed no benefit in announcing it, not with Kacchan’s independent debut so close and the inherent danger of being recognizable current (or former) heroes. Everyone who needed to know already knew, and in some cases, seemed to know much before they did.
Izuku regarded the declaration warily. He didn’t want a freak villain attack to upend their entire lives.

“Izuku,” he growls, slamming the door open.
“Kacchan!” Izuku is beaming, because of course he is, looking delighted and completely unsurprised to see Katsuki glowering in the doorway and gripping his IV stand like a cane. “How are you!”
Katsuki takes in the bandages wrapped around his head, idiotic curls fucked up from sweat and grime, and his dark circles so visible against the white fabric wrapping his face. There’s a wrinkled All Might comic in his lap that he was clearly just reading, and Katsuki doesn’t know whether to laugh or cry at how much of a loser he is. The less honorable part of him wants to put Izuku in a headlock until he cries.
He feels, abruptly, very tired from the walk over. “Move over,” says Katsuki brusquely, approaching his bed.

“I know,” Bakugou huffed. “I was double-checking since you never pass up an opportunity to train.”
“Well, I said no today. Thanks for inviting me, Kacchan.” Before Bakugou could say another word, the door was shut in his face.
“What the hell was that?” Kirishima whisper-yelled.
Deku had been, for lack of a better term, a bitch lately. Which was odd. “Bitchy” was a word people would use to describe Bakugou, never Deku. For someone who was usually the sweetest person ever, he had a temper lately.
“I told you, he’s been a complete dick,” Bakugou sighed.

Or, when Deku begins to act uncharacteristically angry, Bakugou is determined to figure out the cause. He has a guess as to what could be troubling Deku, and he hopes that he's wrong.

Kacchan hummed as he lightly scratched at Izuku’s scalp and massaged the soap through.
Izuku braced himself against the wall. “Why are you doing this?” He sounded wounded—he felt wounded.
“Doing what?”
“Torturing me,” he croaked, resting his forehead against the cool shower tile and silently begging for his torment to end.
Kacchan was giving him glimpses of what he’d never have: careful touches, falling asleep in loving arms, being loved unconditionally, being seen for who he truly was, and being loved for who he was. And it was killing him.

Or, shortly after being discharged from the hospital, Izuku wakes Katsuki up with a bad migraine. Katsuki plays nurse as best he can with one functioning arm and a weak heart. As the days pass and Izuku fails to get better, Katsuki suspects there’s a deeper meaning behind his worsening condition.

Katsuki didn't even realized he was crying until he opened his eyes and watched a tear fall and soak into the mattress. He immediately wiped his in frustration and let out an annoyed sigh before sitting back out, pushing his legs over the bed, and standing up. He knew he said we would stop, that he would learn to calm himself down and be able to sleep alone, but he couldn't take the burning feeling in his heart any longer.
He needed Izuku.
Or
Katsuki can’t sleep due to nightmares and Izuku not being with him and Izuku can’t sleep due to not being with Katsuki and being restless.
