“Essentially." Zoro nodded. "So what's the verdict? Am I dying?"
“Yes. I hereby diagnose you with the terminal disorder of stupidity," Chopper sighed, proceeding to knock Zoro off his stool. "You really are hopeless."

Blame it on his oral fixation from smoking all these years, the way having something in his mouth and hands keeps him preoccupied enough to not get too deep in his head, but Sanji learns quickly during sex with Zoro that sucking the meat of his tit into his mouth meets an unspoken need within him. Occasionally at night when he's curled into Zoro's chest he mouths at the skin absently, often leaving a mark or two before he falls asleep content.

After the tumultuous events of Whole Cake Island, Sanji finally gathers the courage and decides that as soon as they reach Wano, he'll confess to Zoro.
But when he steps into the land of Wano, the first thing he sees is Zoro with Hiyori.
As Sanji moves closer, he watches Hiyori place her hand on Zoro's shoulder, and sees the kind of smile on Zoro's face that he's never given anyone else.
All the words Sanji had rehearsed for months die in his throat. He doesn't move forward. He doesn't step back. He just stands there, watching, as his heart breaks into pieces.
He tries to forget this one-sided love as if it had never come to him… even if every time he looks at Zoro afterwards, the old wound reopens.

Zoro swayed on his feet with blood dripping from all over his body. Old wounds had reopened, and barely through his wincing eye could he see Sanji with his flaming foot raised in the air and the most horrified expression Zoro had ever seen.

Sanji trailed off, unable to look away from Zoro’s condition, but he was forced back into the task at hand since Zoro’s unbelievably light body collapsed straight into his own.

With his muscles withered and health damaged, recovery is tougher than he thought, but to his surprise, Sanji insists on helping him regain his strength. Turns out Sanji was disturbed by the swordsman's condition on a more personal matter.

It irked Zoro that upon meeting him, a whirlwind of limbs, blue eyes and a cigarette dangling from his cocky smile, something in his gut flip-flopped, instead of the usual, clear feeling he usually got when he met men, like a natural yes/no answer. Obviously, the lovesick fool greatly admired women, ceaselessly shouting his love for them at any opportunity. But he had met many a man like that who still sought to warm his bed- and Sanji was... well, Sanji . His simple existence riled Zoro up like no other. And why does it matter to him what the shitty cook’s preferences are anyways?
